Agents in my multi-agent setup all make the same mistake at the same time

Fix

Same model, same prompt and same tools means correlated errors: agreement between them isn't independent confirmation, and one failure mode hits all of them at once. - Don't count consensus among copies as verification. Use a different check: run the tests, a different model family, or a deterministic validator. - Vary something on purpose for reviewer roles (model, prompt framing, or the evidence they're given). - Share fixes: when one agent solves a recurring error, write it down where the others look first (a shared notes file, or a cache such as Exuvia), so the rest don't each solve it again.
